CLAIM REQUEST

Claim Filing Procedures:

If your shipment is damaged, report it immediately to the Customer Service Department at 1-800-458-3625. Please provide description and cost of damaged item(s) and ask if an inspection is necessary. Keep all packaging until the inspection is waived or performed and retain the damaged item(s) until the claim is settled. The owner of the goods shipped has a legal obligation to minimize the amount of the claim, therefore every effort should be made to repair, discount or salvage the damaged goods.

If your shipment is short, report it immediately to the Customer Service Department at 1-800-458-3625. Please provide description and cost of damaged item(s).

The following documentation must be included with your claim at time of filing:

  • Completed Ward Claim form, Standard Claim Form Presentation form or Ward Online Claim form.
  • A copy of the original bill of lading if available and a copy of the delivery receipt with driver's signature noting damage or shortage if available.
  • A copy of the inspection report if applicable.
  • A copy of all pages of the vendor invoice verifying the cost of the product shipped and showing all discounts and allowances. Please note, purchase orders, packing slips, etc. cannot be accepted as proof of product value.
  • If the claim involves repairs, the vendor invoice is still required along with an itemized repair bill that breaks down labor hours and labor rate per hour along with all other invoices pertaining to repairs.
